Unexplained infertility

Hi! I am currently working under the “unexplained infertility” diagnosis after almost 2 years of trying. I believe I might have endometriosis, though I don’t have a lot of the symptoms aside from IBS and lower back pain. I want to do exploratory/excision surgery (if needed). With fertility being my number one priority, my question is do you typically recommend egg retrieval (prepping for IVF) prior to surgery? Or laparoscopic surgery first? Thanks so much!

In cases like yours, where fertility is the main priority and there’s a suspicion of endometriosis, the first step should be a specialized endometriosis mapping (through a transvaginal ultrasound with protocol or MRI) along with ovarian reserve testing, such as the anti-Müllerian hormone (AMH) level. With that information, a personalized decision can be made regarding the best next step.
