I have been told I have a frozen pelvis due to adhesions, fibroids, one of them 10cm, a chocolate cyst 5cm, been told I need a laparotomy with high risk of damage to the bowel. My uterus is also 3 times normal size, and I can’t have any hormonal medication due to breast cancer years ago. Consultant told me he will be getting team together and meeting to plan the operation, etc, waiting on MRI appointment now, but was told I’m very complex case and worry it’s going to be a dangerous major operation. I’m now also having pain in tummy when I empty my Bowels and slight bleeding from back passage. Usually hurts more during a period. I’m just scared this will be dangerous, and how long the recovery time will be, also is painful bowel movements indicate further issues?

You present a complex case of severe deep infiltrating endometriosis with a frozen pelvis, characterized by a markedly enlarged uterus, a 10 cm fibroid, a 5 cm endometrioma, and symptoms suggestive of bowel involvement, such as painful defecation and rectal bleeding during menstruation. Due to your history of breast cancer, hormonal treatments are contraindicated, making surgery the primary therapeutic option. In expert hands, this type of advanced case should be approached via laparoscopy, as minimally invasive surgery performed by a highly specialized multidisciplinary team including advanced gynecologic and colorectal surgeons allows for precise excision of disease, reduced complications, and faster recovery. Preoperative mapping with high-resolution MRI (with vaginal and rectal contrast) is essential to evaluate the extent of infiltration and plan the procedure accordingly. While the surgery is major and the recovery may take 4–6 weeks, when performed by a dedicated endometriosis center with extensive experience in complex cases, outcomes are significantly improved and the risk is carefully controlled.
I can’t have laparoscopy as my fibroids are too large and have frozen pelvis, my specialist informed me that I would be having major abdominal surgery (Laparotomy) with the cut going vertical up my stomach across belly button. How long is recovery for this? And are the chances of me having a stomach after high?
