Endometriosis -excruciating pain

Hi. I’m from Malaysia and I was diagnosed with endometriosis last year . I underwent an ovarian cyst removal op last year. Post op, I developed infection and was treated with antibiotics. I was on 3 cycles I’m depo. Recently had my 2 cycles of menses which was excruciating pain. The pain was extreme. What can or should I do. I work as an air steward and this affects my daily life.

From what you described, it seems that you underwent surgery for an ovarian cyst and later developed a post-operative infection, which was treated with antibiotics. You also mentioned being on three cycles of Depo-Provera, yet the pain during your last two menstrual cycles was excruciating.

When pain persists or worsens after surgery and hormonal treatment, it’s important to consider a few possibilities:

  • Incomplete treatment of endometriosis — Sometimes, endometriotic lesions may not be fully excised during surgery, especially if the approach was more conservative (e.g., cystectomy only without complete excision of peritoneal or deep lesions).
  • Other causes of pelvic pain, such as:
  • Adenomyosis (especially if the uterus is involved),
  • Nerve-related pain (neuropathy or nerve entrapment),
  • Pelvic floor dysfunction,
  • Bladder or bowel-related conditions.

In addition, although Depo-Provera can help some patients, others may experience worsening symptoms or insufficient relief, depending on the type and extent of the disease.

Given your profession as a flight attendant, I understand how debilitating this must be. A thorough re-evaluation — ideally with a specialist in endometriosis — including advanced imaging (like a transvaginal ultrasound with bowel prep or pelvic MRI), review of surgical records, and a multidisciplinary approach may help identify the exact cause of your pain and guide more effective treatment.
