This simple screening tool is designed to help women identify whether they should consider an evaluation for Polycystic Ovary Syndrome (PCOS).

      • Unwanted hair growth

      • Hair thinning or loss

      • Mood changes

      • Infertility

    Please note: This screening is not a diagnostic test. A PCOS diagnosis requires blood work and a medical evaluation by a qualified healthcare provider. This tool can be helpful for pre-menopausal women, as well as for non-specialist providers such as primary care physicians and mental health professionals, to determine if a referral to a PCOS specialist is recommended.
