For endometriomas smaller than 4 cm that do not cause other symptoms, management with progestins such as dienogest is indicated. In some cases, size reduction has been demonstrated.

If endometriomas grow and require further surgery, inhibiting ovulation with oral contraceptives has been shown to reduce the recurrence of new endometriomas.

Isolated endometriomas are rare and are often accompanied by disease in other organs such as the uterus, ligaments, or rectum. If further surgery is required, an imaging study performed by an endometriosis imaging expert should be performed to rule out disease spread.
