Breathwork and Relaxation Training: Utilizing diaphragmatic breathing to regulate the nervous system, improve core function, and enhance pelvic floor relaxation.

Exercise Prescription: Develop tailored movement programs to build strength, flexibility, and resilience while respecting the body’s limits.

Modalities: I pride myself on having the best evidence modalities out there, whether it’s a cold laser, shockwave, or imaging ultrasound for biofeedback, or EMG, among many others. But I still believe the second best tool I have is my hands 🙂 after my ears.

I believe deeply in trauma-informed care that allows the session to be led by the patient’s specific needs. While no two sessions look the same, I often utilize visceral mobilizations, dry needling, breathing for CNS down-regulation, jaw and cranial mobilizations, soft tissue release, internal myofascial release, and somatic release movement techniques. What type of surgery do you perform for endometriosis: Excision

Medication: I will use hormonal medications such as the levonorgestrel IUD, combined oral contraceptive pills, Depo-Provera, or norethindrone for patients who are not interested in surgical treatment or to prevent recurrence after surgical excision in select patients.

Approach to Persistent Pain After Surgery: For patients who have undergone complete surgical excision of endometriosis and continue to report symptoms, I will often explore other pain generators. I do refer patients to pelvic floor physical therapy and perform pelvic floor trigger point and Botox injections, as well as abdominal wall injections to treat myofascial pain. I will also prescribe non-opioid, centrally acting pain medications for patients with very challenging-to-treat pain. If I suspect that the patient’s endometriosis has returned, I will often order imaging in the form of an MRI and potentially offer repeat surgery in appropriate and select patients.

Philosophy: I believe that the etiology of endometriosis is complex and multifactorial. I support the Mullerianosis theory. However, there are other contributing factors such as oxidative stress and inflammation, impaired immune response, genetics, and hormones  – both endogenous and exogenous that play a role in the pathogenesis and proliferation of endometriosis.

Medication: I incorporate medications into my practice for the management of symptoms. For hormonal suppression, I incorporate combined OCPs, progesterone-only pills, or progesterone IUD. Some patients may find that their symptoms are adequately managed with hormonal suppression alone, whereas others may need a second look if symptoms start to recur after their initial surgery. By integrating a multifaceted approach, I find that patient outcomes are overwhelmingly more positive.

Philosophy: Current scientific evidence points to the coelomic theory of endometriosis

Medication: The approach to endometriosis has to be holistic and address not only the disease itself but also potentially associated conditions. At this time, since we do not have medical candidates for curing endometriosis, the first essential step in the treatment of endometriosis is excision surgery. Additionally, Dr. Vidali rely on hormonal contraception, preferably progestins either orally or in the form of IUD to control bleeding or adenomyosis-related factors, if present. He does not rely on GnRH agonists or antagonists as in my experience the risk profile and efficacy profile not better than progestins make them undesirable.

Approach to Persistent Pain: I would like to affirm that I follow all the patients I operate on until they are well. I see the patients periodically as long as necessary. I always have a plan post-operatively especially if history, lab work, and initial examination have highlighted the possibility of the coexistence of additional pain generators. I do recommend physical therapy to most if not all patients and rely on a network of physiatrists.
